How do I add an Income Payroll item if I do not have default items listed?

Income Payroll items can be manually created and the default income items bypassed. This tutorial demonstrates the setup of an Hourly Wage using the manual method. It allows for selecting the calculation type and tehn setting the company specific details to create and activate the payroll item.
