Kentucky

Calculation Type Used:	STATE INC TAX
Fields Required:	State Allowances - number of tax credit claimed
Rate Tables Required:	[TAX] - Kentucky Tax Rate
	[PP] - <Amount> column - Tax Credit Amount
	[PP] - <Constant> column - Standard Deduction Amount

The state of Kentucky doesn't mind the filing status of the employees. Its tax rate schedule is formulated in general for all employees, and that calculation type uses STATE INC TAX.
